PocketRN, a leader in virtual nursing, and Comfort Keepers^®, a top provider of in-home care, have announced a national strategic partnership to deliver the Centers for Medicare & Medicaid Services’ (CMS) Guiding an Improved Dementia Experience (GUIDE) model. This initiative aims to transform dementia care by integrating coordinated medical support, respite services, and community-based in-home care.

As one of nearly 400 organizations selected nationwide, PocketRN will lead Dementia Care Programs, providing families with continuous access to virtual nursing support. Comfort Keepers will complement this by offering specialized in-home dementia and Alzheimer’s care through its franchise network.

Together, PocketRN and Comfort Keepers will expand access to caregiver education and dementia-specific services, helping families navigate the challenges of dementia with dignity, compassion, and hope.
